Purpose
This statement reflects the manner in which information in collected, stored, and used by Pulse Fitness Personal Training Studio.

Privacy Commitment
Pulse Fitness is committed to the protection of your privacy and will take all reasonable steps to abide by this privacy policy. Pulse Fitness supports the Privacy Act and the contents thereof.

Collection of Information
Information regarded as private and confidential may be collected in the following ways:

1. Client Profile forms (used to identify the a person as a client)
3. Medical Health Screening forms (used to ensure that the client is able to participate in exercise sessions without endangering their health)
4. Informed consent forms – (used to inform the client of the potential risks of
exercise)
5. Through the provision of services to clients – (Information that may, for
example, be divulged at the discretion of the client)
6. Use of our website for recording your IP Address, browser type, and
date/time of visit – (used to evaluate the benefits and effectiveness of our
website)
At no time is information sought on the client’s race or ethnic origin, religion,
political association, sexual preference or criminal record.
Use and Disclosure of Information
The above information is collected to allow us to safely (under the OH&S Act)
and legally (under the Commonwealth Privacy Act and the Federal Privacy Act)
to provide a service to you that you have requested.
Personal Information obtained is not released to any other entity or individual
person unless requested in writing by the client. From time to time, we may
provide some statistical information relating to the use of our services to relevant
research bodies but this will not include any information identifying you

personally. Likewise Pulse Fitness Fitness does conduct its own research however
the information collected and published will by no means identify you personally.
Pulse Fitness Fitness may be required to disclose personal information where the
law requires such disclosure.
At any time the client may access their own personal information by contacting
the Manager in writing. This information will not be given to a third person where
Pulse Fitness is not satisfied as to the identity of the person requesting the
information.
If at any time the client feels that their personal information has been
compromised they are entitled to complain to the Manager of Pulse Fitness who will
initially investigate the claim. All complaints must be writing and identify the
conduct that is the basis for the complaint.
If the client finds that their personal information is inaccurate, incomplete or dated
incorrectly please contact Pulse Fitness and it will be corrected at the earliest
convenience.
Storage and Security of Information
Pulse Fitness Fitness is committed to maintaining a secure environment for all
personal information collected. Pulse Fitness will take all reasonable steps to protect
client information from loss, misuse, alteration, destruction, or unauthorized
access or disclosure.
All client information collected is stored under lock and in a secure location.
Information that is stored by electronic means requires a username and
password combination to gain access. Backup copies of our information is
encrypted and also kept in secure locations both onsite and offsite as per our
disaster recovery policy.
Transmission of Information by Electronic means
Sensitive and confidential information will not be transmitted to you over email or
via other online systems. Likewise we will only ever discuss personal details with
you over the phone after verifying your identity by means of a security check, and
with your prior approval.

Exclusions to the Policy
Pulse Fitness may be required to disclose personal information where the law
requires such disclosure.
No information regarding a client is to be released to any person without the
permission of that client. Where no such permission is given, information
regarding clients will only be released upon the serving of a court order.
Changes to this Policy
As it is our commitment to you to ensure your information is kept private, we will
advise, in advance and in writing, any changes to this policy.
